Spesolimab (Spevigo) is available on the NHS as a possible treatment for generalised pustular psoriasis (GPP) flares in adults. It should only be used to treat:
- moderate to severe flares that have not been treated with spesolimab before (initial flares) when:
- the Generalized Pustular Psoriasis Physician Global Assessment (GPPGA) total score is 3 or more (at least moderate), and
- there are new or worsened pustules, and
- the GPPGA pustulation subscore is at least 2 (at least mild), and
- at least 5% of the body’s surface area is abnormally red (erythema) and has pustules, or
- subsequent flares with a GPPGA pustulation subscore of 2 or more (at least mild), if the last flare was treated with spesolimab and improved to a GPPGA pustulation subscore of 0 or 1 (clear or almost clear skin).
A second dose of spesolimab can be used for flares that have not got better (GPPGA pustulation subscore of 0 or 1) after 8 days.
When assessing your GPPGA score, your healthcare professional should consider how your natural skin colour might affect your score and make adjustments if necessary.
